Insurance

We are a participating provider for the following insurance plans.

Our staff will be pleased to submit claims on your behalf. We will collect any applicable co-payments and charges for non-covered services from you at the time of service. We will expect to be reimbursed directly by your insurance company.

If you have insurance benefits provided by a company not listed above, we will provide you with a detailed printed statement that you can use to file a claim. We do require that your balance be paid in full at the time of service. When your claim is processed, reimbursement will be sent directly to you by your insurance company.

Coronavirus Update!

With the coronavirus (COVID-19) outbreak, it is our utmost priority to protect the health and safety of our patients, employees and the community. We are taking the outbreak very seriously.

Following the CDC's recommendations, to help prevent the spread of the virus, we are temporarily closing our office to routine appointments until May 1st.

We are still seeing ocular emergencies and infections by appointment only. We are also here for dispensing of glasses, contact lenses, and pick up of patient's neutraceuticals. Our updated hours are Tuesday - Friday 1:00- 5:00. Please call our office to schedule an appointment at (317) 477-3937.
