We are a participating provider for the following insurance plans.

Our staff will be pleased to submit claims on your behalf. We will collect any applicable co-payments and charges for non-covered services from you at the time of service. We will expect to be reimbursed directly by your insurance company.

If you have insurance benefits provided by a company not listed above, we will provide you with a detailed printed statement that you can use to file a claim. We do require that your balance be paid in full at the time of service. When your claim is processed, reimbursement will be sent directly to you by your insurance company.

We have re-opened! With the coronavirus (COVID-19) outbreak, it is our utmost priority to protect the health and safety of our patients, employees, and the community.

We have limited hours listed below:
Tuesday - Friday: 9:00am - 5:00pm (Closed 12:30pm - 1:30pm for lunch)
Monday: 1:00pm - 5:00 pm by appointment only

When you arrive at the office, please call us at 317-477-3937 and we will open the door for your exam. All patients must wear masks during their exams. Only the patients may enter the office unless they are accompanying a minor. Only one patient and one guardian will be allowed in at a time.

Due to the current CDC guidelines and to limit the exam time for patients, all dilations will be postponed until after June 1st. All patients are recommended to have a retina photo in place of dilation.
